In-Clinic Consent

Please fill out & submit the following consents before your upcoming in-clinic appointment.

CORRECTIVE SKINCARE LA DIAMONDGLOW SERVICE

 

I understand that the DiamondGlow treatment should not be performed within two weeks after receiving any type of dermal injections.

​

I acknowledge that I might experience a scratchy, stinging sensation during the treatment. This sensation will subside during the post-treatment protocol.

​

I understand that if I fail to use sunscreen, I am more susceptible to sunburn and hyperpigmentation.

​

I acknowledge that facial telangiectasia (small blood vessels) is sometimes more apparent immediately after treatment when the skin is thin and will diminish after re-epithelialization (build up of dead cells).

​

I acknowledge that any area around the mouth or face that is prone to cold sores will be avoided during treatment, and that I should avoid all treatments during an outbreak.

​

I understand that my esthetician uses tools that are disinfected and/or disposable.

​

I acknowledge my skin may experience temporary tightness, mild erythema (redness), or slight swelling, which should dissipate in a few hours.

​

I understand that if I am pregnant, lactating, have rosacea, salicylate/aspirin sensitivity, or an outbreak of any skin condition, I should consult my physician prior to receiving the DiamondGlow treatment.

​

I understand that once the desired results are achieved, maintenance is necessary to sustain the results, and it is essential to follow up on all post-care instructions. The frequency and depth depends on the individual’s own genetics, age and lifestyle.

 

These services are cosmetic in nature, and are non refundable. I understand that payment is my sole responsibility.

CORRECTIVE SKINCARE LA DERMAPLANING SERVICE

 

I give my consent for Dermaplaning to be performed by a Corrective Skincare LA Esthetician.

 

Dermaplaning is a physical exfoliation that removes dead skin cells and vellus hair from the surface of the face. This form of exfoliation smooths the skin and allows for the active ingredients in skincare products and treatments to penetrate deeper which increases their efficacy and anti-aging benefits.

 

Alternatives to dermaplaning include microdermabrasion and scrubs for exfoliation and waxing, threading, or cream depilatory for hair removal. There is no single treatment to replace dermaplaning.

 

I understand there are contraindications to this treatment, including but not limited to diabetes, cancer, active acne, bleeding disorders, and the inability for blood to coagulate following injury.

 

Certain medications including blood thinners, higher dosages of Aspirin, and Accutane are contraindicated for this treatment due to increased sensitivity and/or the possibility of delayed clotting from a nick or cut.

 

I certify that I am not taking any of the above medications or experiencing any of the above conditions. I understand this treatment involves the use of a specialized dermaplaning blade to remove dead skin cells and vellus hair.

 

As with the use of any sharp instrument, there is the possibility of injury. While every precaution is taken, I understand the risks and consent to receive the clinic's dermaplaning service.

CORRECTIVE SKINCARE LA CHEMICAL PEEL SERVICE

 

Chemical Peeling is a “skin cell loosening” technique. Skin is supposed to shed and renew itself every 28 days. However, due to genetics and age, this differs in some people and as time goes on the process drastically slows down. A chemical peel is simply an accelerated form of skin exfoliation resulting in newer, smoother skin.

 

Although the name may sound frightening, don’t be scared -- there are many types of chemical peels, varying in strength. There are varying degrees of depth of a peel, ranging from a very light superficial exfoliation (removing dead skin cells and requiring no downtime- like the one we do in a Full Treatment) to a medium or deeper peel (removing more layers for a deeper resurfacing and refinement of the skin with a moderate amount of downtime).

 

A chemical peel is used to help “dry out” active acne (loosening deep impactions making them easier to extract), to help reduce shallow wrinkling and scarring, to help lighten hyperpigmentation (dark spots on the skin), and to help improve the overall appearance and health of aging skin. Clearing depends on diligence to both home care and regularity of treatments. Clearing is also determined by how well instructions are followed.

NOTE: There are no guarantees as to how much the skin will peel.

 

By signing this form, I hereby authorize the chemical peel procedure, should it be recommended.

 

I recognize that there may be: redness, irritation, hyperpigmentation and/or hypopigmentation that may develop after the peel. It may persist for weeks or months and will improve as directions to home care are followed.

 

I understand that once the desired results are achieved, maintenance peels are necessary to sustain the results, and it is essential to follow up on all post-care instructions. The frequency and depth depends on the individual’s own genetics, age and lifestyle.

 

These services are cosmetic in nature, and are non refundable. I understand that payment is my sole responsibility.

No guarantee can be made that the skin will physically flake or peel.

By signing this form, I hereby authorize the DiamondGlow, Dermaplaning, and/or Chemical Peel Service(s), should it be recommended by my Corrective Skincare LA Esthetician.
